How to Apply for a Birth Certificate in Illinois

Records of births in Illinois are maintained by the Vital Statistics Division of the Illinois Department of Public Health. An application for a birth record search can be submitted to the Vital Statistics Division by the person named on the record, a parent listed on the birth certificate or a legal guardian. Certified copies of the original birth record are available, as well as short-form informational copies. Submit an application for a birth certificate in Illinois by mail, fax or in person.

      Download and print the application at the Illinois Department of Public Health website. Mail a written request for an application to the Department of Public Health if you do not have access to the Internet or a printer.

      Select the type of birth certificate you are requesting by marking the appropriate box.

      Provide information as listed on the birth record such as the name on record, parents' names and the date and place of birth, as well as the applicant's personal information.

      Indicate the address where the birth certificate should be mailed.

      Make a photocopy of the applicant's valid photo ID if you are submitting your application by mail or fax. Acceptable photo identification includes a driver's license, state-issued ID card, military ID or a U.S. passport.

      Locate two other documents if you are unable to provide a photo ID, such as a piece of mail with your name and current address and an insurance card, car registration, bank statement or printed pay stub.

      Photocopy both documents if you are mailing or faxing your application. Originals will not be returned.

    Submit Your Request

      Mail your application and a copy of your photo ID to the Illinois Department of Public Health, Vital Records Division. The address is: Illinois Department of Public Health; Division of Vital Records; 925 East Ridgely Ave.; Springfield, IL 62702-2737. Enclose a check or money order payable to Illinois Department of Public Health. Certified copies are sent by mail within three to four weeks of your application.

      Fax your application to the Vital Records office at 217-523-2648. Include a copy of your photo ID and a cover sheet that contains your credit card information, phone number, return address and signature. The processing time for faxed applications is three to five days.

      Drop off the application and payment in person at the Vital Records office in Springfield. Show a valid photo ID and pay the required fee. Birth certificates are mailed within two days of submission of your application.
