How to Get a Child Identification Card
A child identification card can give the police all information necessary to identify her should she go missing. This card contains a picture of your child as well as her full name and other personal information. For additional safety, there are kits available online that will take an imprint of your child's teeth, a copy of her fingerprints, a sample of her DNA, and list any medical information.

Bring your child to the state DMV office and request an identification card. Many states will provide an identification card for any child under 15. Present her birth certificate, Social Security card and your state-issued identification card. Complete the required identification form. The form will ask such questions as your child's full name, date of birth, Social Security number, address and age. The representative will take your child's picture and thumb print. Pay for the identification card. The fee varies per state. The child identification card will contain the child's photograph, customer number, date of birth, height, gender, expiration date and organ donor information. Keep the card in a safe place, such as a locked safe, especially if you won't use it daily. Consider making a copy of it and storing it in another safe location.
