How To Obtain Nebraska Birth Certificates

Few people still have the original copy of their birth certificate. In Nebraska, birth certificate requests are handled by the Department of Health and Human Services' Vital Records office. If you or an immediate relative were born in the state of Nebraska, you may apply for a certified birth certificate copy in one of three ways.

      Apply in person. Download a copy of Nebraska's Application for Certified Copy of Birth Certificate (see Resources for a link to the form). Fill it out completely and accurately. Write a check or money order for $12 per birth certificate payable to ̶0;Vital Records.̶1; Bring your application and payment, as well as your government-issued ID to: 1033 O Street Suite 130, Lincoln, NE 68508-3621.

      Apply by mail. Send your Application for Certified Copy of Birth Certificate, check or money order, an enlarged copy of your government ID and a self-addressed stamped envelope to: Vital Records, PO Box 95066 Lincoln, NE 68509-5065.

      Apply online. Visit Nebraska's birth certificate application form page (see Resources). Fill out the form completely and accurately. Pay the fee with a Visa or MasterCard. Scan your government-issued ID and e-mail it to: [email protected], or fax an enlarged copy to: 402-471-8230.
