The state of Hawaii offers several ways you can request a certified copy of a birth certificate. It can be requested online, in person, or through the mail. A fee is required, and there are certain limitations on who may request the information online. All requests are handled through the Vital Statistics section of the state's Department of Health.

Visit the website of the Online Vital Records Ordering System at Hawaii's Department of Health. Only the parent or individual listed on the birth certificate is authorized to order copies. By filling out and submitting the form online you affirm the information you supply is the truth and that you are authorized to request the information. Fill out all the needed information, including your current mailing address and details pertaining to the person whose birth certificate is being requested. Have your credit-card payment information ready. Ordering a certified copy of a Hawaii birth certificate requires a fee to cover the cost of the document and locating the original. The fee is $10 for the first copy and $4 for each additional copy. Applying online requires an additional electronic processing fee of $1.25 for the first copy and $.25 for each additional one. Apply for a certified copy of a Hawaii birth certificate in person in Room 103 of the Health Department Building, 1250 Punchbowl Street, Honolulu, between the hours of 7:45 a.m. and 2:30 p.m. Monday through Friday. Have a valid government-issued identification card ready to present to the clerk. Fill out the Request for Certified Copy of Birth Record form, also available online. Include accurate information about the person whose birth certificate is being requested. Pay the fee by cash, money order, certified check, or cashier's check.
